Terms and Conditions of Membership
1. In consideration of their mutual promises and subject to paragraph six herein, Crescent Town Club Inc. and the member agree as follows:
(a) Crescent Town Club Inc. grants for the membership period and subject to the conditions of membership set out herein the following rights and privileges (hereinafter called “membership”):

The member represents that they are medically able to participate in and undertake physical exercise and to use facilities and further represents that they are not aware of having any physical, medical, mental or health disability, condition, or disease which might or could result in deterioration of health if physical exercise is undertaken. Each member acknowledges that Crescent Town Club Inc. and its representatives and employees offer no guarantee or warranty, written, or verbal, that the programs, advice, or facilities will have any beneficial medical effect or have been approved by any medical authority.
3. Each member hereby releases Crescent Town Club Inc., its related companies, their representative directors, officers, employees, and agents from:
(a) Any claim whatsoever hereafter arising by reason of the member suffering disease, deterioration of health, illness or aggravation of condition of ill health as a result of participation in the programs, acceptance of advice or use of facilities provided by Crescent Town Club Inc., or its related companies.
(b) Any claim for personal injury sustained by the member in, on, or about the facilities of Crescent Town Club Inc. and its related companies, including without limitation any claims for personal injuries resulting from or arising out of negligence of Crescent Town Club Inc., its related companies, their directors, officers, employees, or agents of the negligence of any other person using the said facilities and acknowledges that they use said facilities at their own risk.
4. Crescent Town Club Inc. and its related companies does not act as bailee of the personal property of any member and Crescent Town Club Inc. and its related companies shall not be liable for theft or loss or damage to the personal property and effects of any member.
5. Each member agrees to keep and obey all rules and regulations now in force and acknowledge receipt of a copy of same or in the future prescribed by Crescent Town Club Inc. and the Club shall have the right to revoke any membership for cause, if any member fails to keep and obey all such rules and regulations without refund to the member of monies paid to Crescent Town Club Inc. Each member agrees that while using the facilities they will not conduct themselves in any way which presents a danger to or creates a nuisance for the staff, or the other persons using the facilities, or disrupts the staff in the carrying out of their duties. Crescent Town Club Inc. reserves the right to amend the Rules and Regulations from time to time.
6. Each member will be issues a non-transferable Membership Card when they join the Club. Upon enrolment, each Member agrees to provide valid identification for identity and address verification, their contact information (Name, Address, Email, and Telephone Number), in addition to a photo image of themselves for security verification. All personal information and images will be kept in the Member’s profile and will remain the executive property of Crescent Town Club Inc. for security and administration purposed only. Personal information will not be shared with any outside agencies or organizations. Each Member agrees to swipe their Membership Card each time they wish to enter the Club. Each Member acknowledges that in the event that their Membership Card is lost, stolen, or destroyed, payment of a service charge will be required prior to replacement of the membership Card.
7. While using the facilities, each member agrees to dress in accordance with the rules and regulation now or hereafter imposed.
8. Members may introduce guests to the facilities of Crescent Town Club Inc., subject to the Crescent Town Club Inc. rules and regulations, including
(a) Only two (2) guests allowed at any one time subject to approval of Management in advance;
(b) All guests must accept and agree to comply with all rules and regulations of Crescent Town Club Inc.;
(c) All guests must sign the Guest Sign In sheet and release Crescent Town Club Inc., its related companies, and their respective directors, officers, employees, and agents from all liability and claims before using any facility.
9. The member represents and warrants that children 12 years old and under are not allowed in the building unless directly supervised in a program or by a parent, guardian, or someone 13 years of age or older.
10. It is understood and agreed upon that Crescent Town Club Inc. may impose a service charge, such charge to be reasonable fixed by Crescent Town Club Inc., from time to time, for any transactions contemplated herein which is not honored.
11. The member shall not be relieved of their obligations to make any payment herein agreed to, and no deduction or allowance from said price or fee shall be made by reason of the absence or withdrawal of any member from the facilities or by reason of any member’s failure to attend or use the facilities or by reason of change in the physical condition of any member.
12. Any notice to the member hereunder shall be deemed to have been duly given if given in writing and mailed to the address shown on the face of this agreement unless such member has given Crescent Town Club Inc. notice in writing of new address.
13. Any notice to Crescent Town Club Inc., with exception of any notice with respect to rescission, shall be deemed to have been duly given if given in writing and mailed to the address shown on the face of this agreement.
14. Crescent Town Club Inc. will have the right to release member’s Personal Information (data and/or image) on request only, to relevant Government Peace Officer(s).
